Crew / inspection collusion

Although unusual, instances are known where the familiarity of crew and inspectors leads to inspection not being undertaken at all or incidences of irregularity or incompetence by the crew are not formally reported. The reasons for such collusion may be personal, but cases also exist of inspectors demanding financial payment by the crew in order to avoid reporting an irregularity.

To minimize such collusion, the rotation of work assignments among the inspectorate requires regular review.
